I am a member of the building (block) resiliency team in our building (on our block) composed of your neighbors. In collaboration with (list building management), we wish to identify those households who may require special assistance during an emergency. This includes when building (block) services such as electricity, heat, and air conditioning, may be cut off or when evacuation from our building (block) may be required. If you wish to confirm this request with our building management you can contact them by calling _________ or e-mail them at ______________.

Your answers will be used to provide your apartment or house priority check-in in case of an emergency and/or support in preparing for an emergency.

1. There is someone in my household, who if left alone, would have difficulty rapidly evacuating our apartment/home especially if the elevator were not in service and might require assistance. (Examples include older residents or those who require the use of a cane, walker, or wheel chair to get about.)
